Medication Mix Up?

Honestly, Who would mix up Vecuronium and Versed? The case of RaDonna Vaught makes all of us a bit nervous. Are those vials similar?  I am sure that I made medication errors, especially when I was a new grad but with new policies and procedures and the scanning of barcodes, it makes medication errors much herder than the old days. I believe that issue was, When  a Doctor accidentally kills someone, what happens? . I have seen a major double standard many times and I am wondering, what are your thoughts?
